Client forms will be sent via email once the appointment is booked via the "BOOK APPOINTMENT" button on the home page. 

Please arrive 5-10 minutes before your appointment to go over intake paperwork.

Late clients may not receive their full session time. 

All no show and same day cancellations will automatically be charged a $50 fee to the credit card on file.
